President's Message by Rosemary McCool
CLEAR's 29th annual conference is only days away and I look forward to welcoming you to Denver. Four hundred of our colleagues will be in attendance, an increase on previous years, and a testament to the value of the event and the broader organization. A further one hundred and fifty attendees will participate in the various training programs offered, while nearly thirty exhibitors will highlight resources to assist us as we carry out our vital mission of public protection. Outstanding learning and networking opportunities lie ahead, and I thank Program Committee co-chairs Caroline MacIsaac-Power and Nancy Swetnam, and all those committee members who have worked tirelessly to prepare another excellent conference. The Denver meeting marks the end of my year as CLEAR President and I would like to take a moment to look back on an extraordinary year of achievement and growth for the organization. The accompanying list of activities details the breadth of work being undertaken by CLEAR, but today I would like to draw your attention to several initiatives I consider particularly significant:

Did you miss CLEAR's webinar? Continuing Competence: An Initiative to Measure and Inform
Over 125 people listened in for CLEAR's webinar this week. Presenters Norman Kahn, Council of Medical Specialty Societies; Melanie Brim, Michigan Department of Community Health; and Rosemary Gibson, Robert Wood Johnson Foundation gave perspectives on developing initiatives addressing how the healthcare system will determine, measure, evaluate and assure the public about a physician's competence throughout his or her professional career. Here are some highlights from the webinar:
- The National Alliance for Physician Competence has developed the Good Medical Practice USA document, which describes desirable characteristics of competent physicians. Several states are developing educational tools built around GMP.
- The notion of competence is changing, with a shift to a culture of continuous improvement in medical practice. - 40 + state medical boards now require Continuing Medical Education (CME) credits. Traditional CME has faced recent criticism that it has not been demonstrated to produce changes in practice behaviors. New types of CME include Evidence-Based CME, Point of Care CME, and Performance Improvement CME, including the Improving Performance in Practice model. - The shift to a continuous improvement model of assessment requires medical boards to acknowledge the difference between "knowing" and "doing" and shift from knowledge-based assessment to system-demonstrated performance and moving to practice-relevant assessment and education. - A change in the culture of regulatory boards would include a move to a supportive rather than punitive environment where remediation and education are important. - The National Alliance has engaged thoughtful members of the public in this conversation, realizing that the public and professionals share a common interest in assuring continuing competence.
